Palm Drive Health Care District is a resource for health care services in West Sonoma County. The District’s primary mission is to deliver access to quality, compassionate health services responsive to the needs of our District. We fulfill that mission through our ownership of Sonoma West Medical Center (formerly Palm Drive Hospital), and through partnerships with community-based providers of health and wellness information, classes, services, and other programs.
The District was formed in April 2000 and is a government entity of the State of California. It serves 50,000 people who live in western Sonoma County, including the communities of
Sebastopol, Graton, Forestville, Bodega Bay, Carmet, Salmon Creek, Jenner, Duncan’s Mills, Guerneville, Occidental, Freestone, Rio Nido, Monte Rio, Guernewood Park, Summerhome, and Mirabel Park.
PDHCD is committed to transparency. This web site makes available all of the public documents relating to the operations, funding, and governance of Palm Drive Health Care District.
